Case Management is a Service: a one-on-one relationship for linking people to the resources they need.
The mission of the Turning Point Case Management program is to provide quick response services and community connections for adults in Niles Township who experience challenges in daily living. Case managers and participants customize comprehensive service plans and work in tandem to empower clients to live and grow with lasting positive change.
Some of the areas that a case manager can help with include connecting clients with resources, like food pantries, shelters, and applying for entitlements like food stamps, cash assistance, and health insurance. Case Managers can also help clients with the Social Security Disability application process, and can accompany clients to some related appointments. A Case Manager may accompany a client to the local DHS office to work out issues with Medicaid coverage, and related spenddown issues.
We believe that adults in these circumstances are best served when a compassionate professional responds quickly and works with the client to comprehensively asses their short and long term needs. We believe that one of the greatest signs of respect is to underscore a client’s strengths and potential, even in a time of crisis, by seeking an appropriate balance for the case manager and client to share the action taken on the clients’ behalf.
